To Travel to Cuba, a Visa is mandatory. If you were born in Cuba and wish to obtain a Visa, please print the Visa Application Form; sign and fill out completely. Please don't leave any blank spaces. Print one copy each and sign the two following forms: Travel Affidavit and Affidavit. Forward all forms (3) to the address below, along with: · 2 passport pictures · A copy of your Alien Resident Card or American passport · A copy of your Cuban passport (If you came from Cuba before 1970, you don't need one) · Payment of $125.00. Money Order or Personal Check. *All copies must be clear and all numbers and pictures must be legible.
If you were born in Cuba and have your Permanent Visa (Vigencia de Viaje), print and sign the Travel Affidavit and Affidavit. Forward both forms to the address below, along with: · A copy of your Cuban Passport · A copy of your Alien Resident Card or American Passport · A copy of your Permanent Visa (Vigencia de Viaje)
If you were born in the United States, please print and sign the Travel Affidavit and Affidavit. Forward both forms to the address below. Include a copy of your American Passport. If you were born in another country, please include your Alien Resident Card and copy of your country's passport. Include your payment of $50.00 (Money Order or Personal Check).
